COORDINATED LONG TERM SERVICES – CoLTS

This program is for any Medicaid eligible person that requires nursing home level of care

PERSONAL CARE OPTION

This is a program for eligible Medicaid recipients over 21 who have a medical need for services. This program provides a

caregiver that may be chosen by the client to assist with the activities of daily living, keeping them independent in their home.

 

CoLTS C WAIVER

This is a program for eligible Medicaid recipients. Nursing services and respite care for family members are available. Personal

care attendant are offered for this program under the pco program.

MEDICALLY FRAGILE WAIVER PROGRAM

This program is for eligible Medicaid children and adults who are at risk for or have a developmental delay or disability and a

medically fragile condition. Home health aides and skilled nursing are offered.

EARLY PERIODIC SCREENING, DIAGNOSIS & TREATMENT (EPSDT) PROGRAM

This program is for eligible Medicaid recipients under 21. Personal care attendant

for daily living assistance is offered.

CLIENTS NEED NOT BE HOMEBOUND TO QUALIFY!
